What we do:
YAP advocates meet with youth and families at times and days that are convenient for them to help them execute tasks or participate in community-based activities based on the individual plan. Staff are available 24/7 to the families and work to promote positive environments and outlets for the youth, with the overarching goal of avoiding out-of-home placement.
Who we are:
YAP is a strength- based national organization that works as a team with families and other supports to develop achievable goals that reflects the passions and needs of the youth we work with. YAP believes in child center Individualized Service Planning that focuses heavily on community engagement and positive experiences.

Is a referral needed?
Yes. To be enrolled in YAP, a family must also be currently working with Essex County Department of Social Services. There is a permission to contact form that any family, service provider, or school can complete to initiate communication with YAP before an official referral, or work with D.S.S.; begins.
Where can the referral form be obtained from?
The Referral can be picked up at the YAP office 222 Water Street, Elizabethtown, NY or can be requested electronically through Amanda Douglas at amdouglas@gmail.com
