We provide in home, community, and certified setting programs and services to those who reside in Essex County, New York.

Programs and Services:

Residential

Supervised Residences:

Mountain Lake Services supports individuals in supervised residences throughout the Essex County. Supervised Residences provide 24-hour supervision. Each person enrolled in the service is provided with the support to pursue their individualized interests and desires, with an emphasis on personal choice. 

Supportive Apartment Program:

Mountain Lake Services Supports 8 individuals in Supported Apartments in Essex County. The apartments are OPWDD certified, and 24-hour supervision is not required. The overall goal is to assist individuals to live as independently as possible and become an integrated and productive part of their communities.  

Cornerstone Program:

The Cornerstone Program was created as a best-practice treatment program for individuals with intellectual and developmental disabilities and sexual boundary violations, living residentially at Mountain Lake Services. The Program offers individual counseling, weekly treatment groups, and therapeutic environments in home services settings. The participants have Risk Management Plans to ensure personal and community safety. All individuals in the program are screened for risk manageability by trained clinicians and through consultation with an international consultant and are based on a philosophy of reduced supervision rather than containment. 

Day Habilitation: 

Day Habilitation is provided at several locations in Essex County. The program focuses on independent living skills, recreational opportunities, volunteer work, and community involvement. The environment encompasses activities based on individual strengths, desires, and skill level. Services can be provided on a certified site (Site- Based Day Habilitation) or in the community (Day Habilitation Without Walls). 

Community Habilitation:

This program offers services inside the home and in the community, allowing people to maintain their independence within their community.

Individual supports & Services:

This is a reimbursement program available to individuals who are living in their own home or apartment in the community. This program allows people to maintain their independence. 

Article 16 Clinic: 

The Article 16 Clinic provides therapy services to individuals and their collaterals which are habilitation or rehabilitation in nature at six different locations located throughout the Tri- County area. Specializing in Physical Therapy, Occupational Therapy, Speech Therapy, and Mental Health Services which includes Psychotherapy and Psychiatry services. These services occur at certified clinic sites and provide quality services using experienced and licensed staff. 

ACCES-VR:

This program assists individuals to achieve and maintain employment.

Respite:

Respite provides a reprieve for caregivers. This service can be provided at home or in the community. In addition, there are also respite beds located throughout the agency where overnight respite can be provided at our homes. 

Community Pre-Vocational Services: 

This program provides skill building for individuals who want to work but face barriers. This program starts with discovery of the individual’s interests and then setting up volunteer opportunities along with exploring different job settings and fields. 

Supported Employment:

This service places people into integrated work sites in the community with support of a Job Coach. The Job Coach gives intensive on-site training and provides individuals with support to ensure successful long-term employment. 

Fiscal Intermediary:

Fiscal Intermediary Services support those who self-direct their individualized budgets by completeing tasks such as billing and payment of approved goods and services, fiscal accounting and reporting, Medicaid compliance, and general administrative supports.

Family Supports and Services: 

This program provides planned and coordinated services to families who have a developmentally disabled family member living at home. This program helps to alleviate the pressures experienced in home setting. The goal is to assist these families to remain intact for as long as they desire and avoid unnecessary out of home placement. This service is available in many different forms including Reimbursement, Mountain Lake Kids Camp, Autism Behavioral Services and Crisis Services, and Intensive Behavioral Supports.
